The client and agency assessor will develop a Care Plan during the initial Intake and Needs Assessment. Providers visit your home to provide service according to your Care Plan.
In-Home Services Personal Care is a cost share program with clients contributing between $3 and $20 per hour. Provider substitutions are offered as available.
- Supervision, socialize, companionship
- Play cards, games, crafts (client provided)
- Read to the client
- Walks
- Assist with light meal preparation and/or feeding
- Light homemaking and/or personal care (as approved)
